Each client’s story is a snapshot of Medicare Rights’ more than three-decade commitment to ensuring that older adults and people with disabilities can get the affordable health care they need.

Please do not provide any identifying information other than what is asked on this form. Examples of information you should not provide include, your Social Security number, Medicare number, address, or other sensitive information.
Your stories will only be used by the Medicare Rights Center to highlight the effectiveness of the Medicare program, and to advocate for meaningful changes to help improve the program. Your name, contact information, and story will not be shared with any third party without your consent.
